- Current CV/Resume
- Go to http://www.fsmb.org and obtain your credential verification. Print the certificate from the website or request it.
- Zimbabwe Philanthropic Policy and Application Form
- Copy of your current license(s)
- Copy of your Medical School Diploma
- A photocopy of face page of passport
- Current CV/Resume
- Mail 2 Official passport pictures to the address listed below.

Information
- Make sure there are at least 6 free pages and your passport is NOT expired until at least 3 weeks AFTER the mission is completed.
- If you have a name change, provide proof of this name change- i.e. marriage license etc.
